Here is a link to test your knowledge: http://empowern.com/2015/08/bipolar-medication-lithium/ Lithium belongs to the drug class: antimanic agents and it is sold under the US brand names: Lithobid, Eskalith, Eskalith-CR. More than 5.7 million American adults or 2.6 percent of the population age 18 or older in any given year have bipolar disorder. Lithium is helping some of this American. It has become one of the first and widely used psychiatric drugs for the treatment of bipolar disorder in the United States, since its approval in the 1970s. It also has become one of the most studied psychiatric medications. Indication: Lithium is an effective drug for bipolar disorder as its properties help to prevent, as well as stabilize, manic-depressive mood swings that occur in this psychiatric condition. By controlling mood disorder, it also reduces the risk of suicide. This is an advantage of lithium that is lacking in some other drugs. But it is more effective in preventing depression than it is in preventing mania. However, it needs time to develop and reach its ideal therapeutic level in the body before it induces any effects. Usually it takes about 1-3 weeks, however, in the case of sudden or acute bipolar episodes, if lithium hasn’t reached its optimum level in the client’s body, it can be prescribed along with other medications for effectiveness. Lithium can also be prescribed for other psychiatric disorders besides bipolar disorder. It can be useful when other drugs fail in conditions such as schizophrenia, major depression, and certain psychiatric problems in children. Mechanism of Action: Although lithium had been used for more than 50 years in the treatment of bipolar disorder, its mechanism of action was unknown; until 1998. Scientists didn’t know the cause of mood swings in bipolar disorder either. A study made in 1998, by a group of researchers from Wisconsin University shed light on how lithium works and why it is effective. It was concluded that lithium acts on certain nerve cells in the brain and the receptors of the neurotransmitter glutamate. Lithium has a dual effect on these receptors, and using this, it helps to regulate the level of glutamate between cells, preventing it from reaching too high or too low concentrations. According to the director of this research, Dr. Lowell Hokin, who is also a school professor of pharmacology at Wisconsin University; too little glutamate between neurons cause depression, while excessive amounts of glutamate cause mania. But it is also believed that there are more causes and factors involved in these disorders besides the level of glutamate, but nevertheless, the study has been very helpful as it has uncovered the fundamental causes of bipolar disorder.

Symptoms of schizophrenia can be broken down into 2 categories, Positive and Negative Symptoms. Positive symptoms include behaviors or sensations that are not normally present. Examples include hallucinations, delusions, and catatonia. These symptoms are thought to be related to an excess of dopamine. I remember this by remembering that “doPamine has a P in it”. So P for Positive and P for Dopamine. Negative symptoms are the absence of normal behavior. Examples include a lack of initiative, diminished speech, disheveled appearance & flat affect. These symptoms are thought to be related to an excess of serotonin. As we will see antipsychotics affect dopamine and serotonin to varying levels. The indications for this class of drugs includes psychosis (mainly schizophrenia), Mania (mainly bipolar disorder), aggression and Tourette's disease. Typical Antipsychotics primarily block dopamine receptors in a non-specific manor. Therefore, these drugs work best for positive symptoms, and have little effect on negative symptoms. The non-specific mechanism of the drug also means there are lots of side effects. Some of these medications come in a slow release injectable form so they can be used in non-compliant and aggressive patients. There are a lot of high yield side effects so we will break them down one by one Neuroleptic Malignant Syndrome (or NMS) is a rare but potentially fatal adverse reaction of typical antipsychotics. It involves fever, altered mental status, rigidity and autonomic instability (such as tachycardia, hypertension, diaphoresis etc.). You may also see elevated myoglobin in blood or urine and elevated Creatine Kinase (CK). One of the ways I think about it is that it looks kinda sorta like Serotinin Syndrome that you can see with antidepressatns. If you see this you have to emergently stop the medication, provide supportive care and consider adding Dantrolene Extrapyramidal Symptoms (or EPS) are due to blockage of Nigrostriatal dopamine. It can present with a number of different symptoms. Akasthisia is a general sensation of restlessness Acute Dystonia is involuntary continuous muscle contractions often of the neck. Another common presentation of acute dystonia is Oculogyris Crisis when your eyes get locked looking upward and you have to lean over to see Dyskinesia (AKA Pseudoparkinsonism) presents like Parkinson’s Disease with symptoms like a pill rolling tremor, cogwheel rigidity & bradykinesia (or slow movement) Tardine Dyskinesia (or TD) is uncontrollable facial tics, grimacing & tongue movements As scary as these symptoms may look, they are generally not medical emergencies. In most cases you will continue to use the drug with perhaps a reduction in the dose or the addition of an anticholinergic mediation like Benzatropine or Diphenhydramine. Tardive Dyskinesia is the exception and requires cessation of the medication as it can be permanent. Usually you would switch a patient with TD to a 2nd gen antipsychotic. Hyperprolactinemia is a side effect due to Blockage of Tuberoinfundibular dopamine. It presents just like any other disease that increases prolactin. So you can have galactorrhea, gynecomastia, decreased libido and menstrual irregularities. The text for this video is too long and exceeds Youtube max allowed length. The National Bureau of Economic Research (NBER) reports that there is a “definite connection between mental illness and the use of addictive substances” and that mental health disorder patients are responsible for the consumption of: 38 percent of alcohol 44 percent of cocaine 40 percent of cigarettes NBER also reports that people who have been diagnosed with a mental health disorder at some point in their lives are responsible for the consumption of: 69 percent of alcohol 84 percent of cocaine 68 percent of cigarettes There’s clearly a connection between substance abuse and mental health disorders, and any number of combinations can develop, each with its own set of unique causes and symptoms. When you have both a substance abuse problem and a mental health issue such as depression, bipolar disorder, or anxiety, it is called a co-occurring disorder or dual diagnosis. Dealing with substance abuse, alcoholism, or drug addiction is never easy, and it’s even more difficult when you’re also struggling with mental health problems, but there are treatments that can help. With proper treatment, support, and self-help strategies, you can overcome a dual diagnosis and reclaim your life. Alcohol or drugs are often used to self-medicate the symptoms of depression or anxiety. Unfortunately, substance abuse causes side effects and in the long run worsens the very symptoms they initially numbed or relieved. Alcohol and drug abuse can increase underlying risk for mental disorders. Mental disorders are caused by a complex interplay of genetics, the environment, and other outside factors. If you are at risk for a mental disorder, drug or alcohol abuse may push you over the edge. Alcohol and drug abuse can make symptoms of a mental health problem worse. Substance abuse may sharply increase symptoms of mental illness or trigger new symptoms. Alcohol and drug abuse also interact with medications such as antidepressants, anti-anxiety pills, and mood stabilizers, making them less effective.

Brain stimulation effective with eating disorders

(16 Nov 2011) This is repetitive transcranial magnetic stimulation, or rTMS. Put simply it's a high frequency magnetic field that is sent through the skull. A twitch in the fingers of this student volunteer shows that the target part of the brain is being reached. These connections are vital for example to reach the dorsolateral prefrontal cortex, which is implicated in the experience of craving, and an important trigger for bingeing. Dr Frederique Van den Eynde is part of a team which is investigating whether this type of stimulation may one day be used to treat people with eating disorders. Particularly anorexia nervosa. This sort of brain stimulation is already being used to treat depression, but it's not yet an accepted therapy for food disorders. Kat Damezer was at school doing exams when she became anorexic. During a family holiday to Tibet she caught a virus and became very ill, losing a considerable amount of weight. At first she enjoyed the weight loss. Then she enjoyed the sensation of being hungry and she liked the fact that she was destroying her body. Damezer says "I used to feel faint all the time and every morning black out for a while every time I got out of bed and I used to be freezing, constantly freezing and shivering and having funny heart palpitations and having this constant feeling like your stomach was eating itself, you're just so hungry and it kind of becomes enjoyable and satisfying because you know you're doing a good job at having an eating disorder." Doctors say anorexics are the greatest danger to themselves. They are intensely controlled, focussed and able to use many deceptions to cover the fact that they're not eating. Damezer was intellectually very bright and destined to become a medical student. But she became so thin that her teachers gave her an ultimatum: see a doctor, or leave school. Within days Damezer was admitted to hospital. She was losing weight rapidly and it was having a severe impact on her heart. It would be seven months before she was allowed to leave the ward and much longer before she could resume her studies. With others in the eating disorders unit she had to learn to eat again and to feel normal around food. Then came psychotherapy and after a diagnosis for depression she was prescribed antidepressants. An insight to that time is revealing. Damezer remembers her shock at seeing a new girl on the ward who was thinner than she was "I remember first seeing her and kind of being taken aback by her, but then just being jealous knowing that I wasn't as thin as her, at the same time knowing that it didn't look good, but still being jealous." She says "I saw my spine in the mirror and thought, I was quite scared because I could see all the bones protruding and it was the only point when I thought, oh it doesn't look too good. Apart from that I knew I wasn't attractive to other people, but it was unacceptable for me to look any other way really." According to the Institute of Psychiatry about 2 per cent of women will be affected by anorexia nervosa during their lives. The mortality rate is high. A fifth will be killed by their illness. Professor Ulrike Schmidt is leading investigations into potential treatments at the Institute of Psychiatry and she believes anorexia has a long history reaching back to the middle ages. She says although there are important overlaps between different eating disorders, like attitudes to image, shape, weight and appearance, anorexia has important differences. It was described clearly in the 19th century whereas other types of disorder like bulimia are more modern, western, culture bound phenomenon linked to anxiety. Schmidt believes bulimic patients may have triggers like a trauma, or depression. Gel Fights Breast Cancer With Fewer Side Effects

An effective treatment for non-invasive breast cancer. One in 8 women will be diagnosed with Breast Cancer in their lifetime. You have probably either had, will have, or know somebody who has had breast cancer. Why one person gets the disease and another doesn’t is not completely known. What doctors do know is that breast cancer can be caused by damaged DNA inside a cell. That cell mutates and multiplies over and over. The good news is earlier detection, increased awareness, better treatments and technology means it’s more likely than ever before patients will survive it. “Many women have a phase called non-invasive cancer,” said Seema Khan, a surgical oncologist at Northwestern University in Evanston, Illinois. Non-invasive cancer is confined to a woman's milk ducts or breast lobes and has not spread beyond the breast tissue. Anti-hormone drugs are a common treatment for this type of breast cancer. And now, oncologists at Northwestern are developing a gel for the skin that goes on the outside of the breast to kill the cancer. The gel is made out of the drug, Tamoxifen — that’s already used to treat and prevent breast cancer. “You could put Tamoxifen in a gel and apply it to the breast skin, and it would get through the skin in sufficient concentrations that it would actually do what it needed to in the breast, but the exposure to the rest of the body would be low,” explained Khan. Unlike other typical cancer medications, the gel treatment is confined to the breast—lowering the risk of damage to other organs. “There are many reasons to expect that this would be a really positive change in the way we deliver drugs to the breast for these early problems,” said Khan. Early studies have found the gel form of Tamoxifen stopped cancer cell growth just as well as tamoxifen pills and caused fewer side effects. It could be several years before the gel is on the market. But early study results are giving it the thumbs up in the fight against some non-invasive cancers.

I HAD SEVERAL uninformed people comment that metformin was unsafe, and claimed that metformin was actually bad for your kidneys. Since metformins introduction and widespread use, Kidney failure in diabetics has actually gone DOWN 28.3%, while in non-diabetics, it has gone up 65%.Heart attacks have also gone down 68% as compared to a 31% reduction in non-diabetics, likely almost exclusively from statin drugs like Lipitor and Crestor. But there's another type of vaccine refusal, and it's important that we not ignore that, too. Doctors sometimes promote the use of some vaccines with less enthusiasm than others. Sometimes, they don't talk about them at all. This occurs most often with the herpes simplex virus, or HPV, vaccine.
