My experience with antidepressants

The term unipolar refers to the presence of one pole, or one extreme of mood- depressed mood. This may be compared with bipolar depression which has the two poles of depressed mood and mania (i.e., euphoria, heightened emotion and activity). Bipolar disorder (also known as manic depression) causes serious shifts in mood, energy, thinking, and behavior—from the highs of mania on one extreme, to the lows of depression on the other These are of 2 types • Bipolar I disorder involves periods of severe mood episodes from mania to depression. The highs may even require hospitalization of the person. • Bipolar II disorder is a milder form of mood elevation, involving milder episodes of hypomania that alternate with periods of severe depression The depression that people with bipolar disorder experience is generally of a melancholic or psychotic type and therefore more biological in its nature. The depression found in these bipolar patients can be difficult to treat than unipolar depression as the antidepressants can cause them to have greater mood swings. These patients need to be on mood-stabilizing drugs. The antidepressants alone can actually increase the manic episodes and worsen the disorder. Antidepressants: divided into three main classes: • Selective serotonin reuptake inhibitors (SSRIs) - were developed in the 1980's and are the most common prescribed today. They are sold under brand names such as Prozac, Paxil, Prozac, Luvox, Zoloft, Celexa • Monoamine oxidase inhibitors (MAOIs) There are three types of MAOIs, phenelzine,(Nardil) isocarboxazid and tranylcypromine, ( Parnate) and moclobemide.) • Tricyclic drugs (TCAs). (sold as Amitriptyline, Imipramine) For effective control and to limit the side effects a combination can be used such as Wellbutrin (or bupropion). Mechanisms of action are different hence lower doses of both help control side effects. The combination of Zoloft and wellbutruim earned the popular name Welloft. The MAO inhibitors are more effective than the SSRI drugs. But can cause severe weight gain and therefore patients need to work on diet control, else the spike in blood pressure can be dangerous. They are contraindicated with drugs that work on serotonin. Lamictal (or Lamotrigine) was FDA approved for treating depression in 2003.The main advantage being, no sexual dysfunction, no sexual side effects and no weight gain. There is no sedative effect and dosage is generally much less than used for seizures. If any of the above mentioned combinations are ineffective in controlling depression, the thyroid hormone replacement drug Cytopan or Cytomel or Synthroid can be added to any of the above drug. Atypical anti-psychotics such as low doses of Zyprexa, or Abilify, or Seroquel, and the latest Quetiapine can also be prescribed. 150 mg of Lithium, normally prescribe for Bipolar at much higher doses, can be used safely without having to get regular blood checks. Mirtazapine is a tetracyclic antidepressant in the NaSSA class. It has a similar efficacy to SSRIs and TCAs, and it may be a better option for certain populations. Serotonin, or 5-hydroxytryptamine (5-HT), is a neurotransmitter involved in many brain and body functions and is commonly known as the substance of well-being and happiness. Serotonin is produced in specialized neurons found mostly in the Raphe nuclei located along the midline of the brainstem. The axons of these neurons form extensive serotonergic pathways that reach almost every part of the central nervous system, including the cerebellum and the spinal cord. This is why it’s not surprising that serotonin is implicated in a vast array of brain functions, including sleep and wake cycle, appetite, mood regulation, memory and learning, temperature control, … among others. Serotonin is synthesized from the amino acid tryptophan and is stored in small vesicles within the nerve terminal. When a serotonergic neuron is stimulated, serotonin is released into the synaptic cleft where it binds to and activates serotonin-receptors on the postsynaptic neuron. Serotonin action is then TERMINATED via removal of its molecules from the synaptic space. This is accomplished through a special protein called serotonin-transporter. Low levels of serotonin in the brain have been associated with depressive disorders and current treatments for depression aim to increase these levels. The most commonly prescribed medications, called “selective serotonin reuptake inhibitors”, or SSRIs, act by blocking serotonin reuptake by the transmitting neurons. This results in elevated levels of serotonin in the synaptic space and its prolonged action on the receiving neuron. The SSRIs have developed into the drugs of choice because they produce fewer side effects thanks to their selective action on serotonin alone and no other neurotransmitters. Unfortunately, because serotonin is involved in a wide range of brain functions, the side effects remain significant and may progress to a potentially dangerous condition known as "serotonin syndrome". This syndrome is generally caused by a combination of two or more drugs used to raise the serotonin levels in the brain. If the medications are not discontinued, the condition may become fatal. Nonpharmacologic methods of raising brain serotonin have shown promising results in recent studies. It has been suggested that positive mood induction, either self-induced or due to psychotherapy, correlates with INCREASED serotonin synthesis in the brain. The interaction between serotonin synthesis and mood may therefore be 2-way, with serotonin influencing mood and mood influencing serotonin. Other methods include exposure to bright light and tryptophan-rich diets. To note, however, that serotonin-rich food such as bananas would NOT work because serotonin, unlike tryptophan, can NOT cross the blood brain barrier. Finally, although it sounds like a cliché, physical exercise maybe the most effective and safest way of improving mood. Several studies suggest that serotonin levels are increased with vigorous physical activity and that these elevated levels are maintained for several days after the exercise.

The medication is inexpensive and readily available, and it is well-tolerated in patients treated for depression. Because knee OA has no known cure, pharmacologic interventions are focused on pain relief and promotion of function. Initial management consists of patient education, exercise and weight loss, but many patients also require analgesics typically in the form of non-steroidal anti-inflammatory drugs (NSAIDs) or opioids. NSAIDS are well known to reduce pain, but they also significantly increase the risk of gastrointestinal ulceration and bleeding, and are associated with cardiovascular events. Thus, their long-term use is contraindicated in many patients. Opioids, in some studies, have been no more effective than placebo for knee OA patients. Recent trials of the serotonin and noradrenalin reuptake inhibitors (SNRIs) venlafaxine and duloxetine have shown statistically and clinically significant reductions in pain in patients with OA. I lost 25 lbs in 4 weeks and was in full panic or near panic for 8 weeks straight mixed with the darkest most painful depression I cold have ever imagined. Weeks after starting my newest round of medications (Seroquel & Nortryptaline) as a last ditch effort, I had my blood drawn for possible hormone imbalances and my Testosterone levels came back 200 ng/Dl and 150 ng/Dl. The average 25 year old male has 750 ng/Dl. With this discovery I for the first time had any type of possible explanation as to why I was not getting better and why I might be so so sick. The symptoms of such Low T are very similar to those of major depression. I started Testosterone replacement therapy soon after and have been checking in with the world and documenting my experience with treatment as well as giving my insight and perspective on various topics of mental health. I am blessed to say that I have slowly, over the last 4 years, been improving and becoming more stable which I never thought to be possible. My low T manifested itself in the form of Major depression, anxiety, and depersonalization/ derealization for over a year. Gaining some mental stability back is nothing short of a miracle as I was near death for what felt like forever. I do not consider myself to be totally healed yet but I am closer now then ever before and aim to use what I have been through to help or at least offer support to others in need I was able to successfully come off my Seroquel and Pamelor. I work out all the time as a part of my mental health recovery!!! Weight training and all kinds of cardio rule much of my free time!

The overdose of any medicine is bad. It is not confined to sleeping pills. The research which brings out this medicines and tells us what dose to be used and how it is to be used. The overdose of sleeping pills can be the result of any psychosocial problems to any individual. The side effects of the sleeping pills, like Alprazolam, there can be first CNS manifestation. The medicines are supposed to be a sedative, hypnotic or anti-anxiety in many ways so we will use Alprazolam very commonly but nowadays it is a prescription based drug. So it is not available freely over the counter. If a person is having a certain number if pills for a certain diseases then there can be an overdose and it can lead to neurological manifestations and cardiological manifestations. There can be conduction disturbances in the heart. There can be excessive somnolence and the person cannot wake up from the sleep. There can be disturbance in the sensorium, can have memory problems and can have gastrointestinal side effects of this medicines.

Should be pretty familiar with this drug if you're getting into advanced care, paramedicine, because it is a very widely used medication for a few different reasons. Okay? And so the whole idea of benzodiazepines, like you saw with barbiturates, if you've gone through that video yet, is that benzodiazepines affect the GABA receptors. And so GABA receptors are the main inhibitory neuron or neurotransmitter that's within the central nervous system. And so when it produces a response, or benzodiazepine attaches and produces a response from the GABA receptors, it causes a decreasing level of consciousness. Okay? That's essentially the whole point of having benzodiazepines is that what it does within the GABA receptor, what it does in the central nervous system, is that it slows down transmission of signals. Okay? And so by doing that, it's basically have an influx of chloride. It's hyperpolarizing the cell, and so by hyperpolarizing the cell, it's much more difficult for a neuron to fire. And that's what benzodiazepines are doing is that it's hyperpolarizing that cell so it's difficult for it to fire. And that means that we're going to have less central nervous system transmission, and so that's why it's good for using it for seizures and aggressive patients, anxiety, those kind of things, 'cause those are all due to severe neuron electroactivity within the central nervous system that's producing an affect in the body. Okay? And so that's what benzodiazepines are great for, is that it reduces the amount of electroactivity in the central nervous system to the point where you're going to see decreasing level of consciousness, and if you use enough of it, a full complete unconsciousness with a severe respiratory depression. So that's another side of it is you got to be really careful with how much you use, because of its pretty hard-hitting effects. Okay? So very, very important. So, reduces anxiety. It's a sedative. Anterograde amnesia, convulsing, and muscle relaxant. It does all these things. Okay? It has a lot of different actions. It's a very widely used medication, stuff like Versed and Ativan, both very widely used benzodiazepines. Okay? Big thing with this is to make sure you're monitoring the SpO2. Very, very important because SpO2 is going to be affected just because we have a severe respiratory depression and decrease in central nervous system reactivity, so we might see some desats on these patients, so it's important to put an SpO2 monitor on them, and if they desat to the point where you need to put an oxygen on them, that's important to identify in someone that's using benzodiazepines. And everyone is affected differently with benzos. Typically I go on the low end, mostly because of experiences with, not overdosing, but definitely having pretty major effects by giving someone a little too much. And it wasn't even too much. I mean it's just by the fact that it's a normal thing in a protocol to give anybody 2.5 to five milligrams of Versed, and in a couple patients that have had, I've seen some pretty major respiratory depression, decrease in central nervous system reactivity with using just those types of doses. So typically I use anywhere between one and work my way up to the point where now I have the achieved effect. Okay? Always going lower and working my way up where it's the better way to do it. That way you can monitor and control your patient that much better. Okay? Depending on the effect you're looking for. Okay? There is a way to reverse the effect as well. We don't carry it typically on an ambulance, but Flumazenil is the medication of choice for reversing benzodiazepines. Rarely is it used, but if it is, that's the drug of choice it would be for reversing benzodiazepines. If you have any questions, please email me or post the question in the group, and we'll chat. Vilazodone effect on sexual function in GAD patients - 103408
Video abstract of original research paper “Characterizing sexual function in patients with generalized anxiety disorder: a pooled analysis of three vilazodone studies” published in the open access journal Neuropsychiatric Disease and Treatment by authors Clayton et al. Background: Vilazodone has been shown to reduce core symptoms of generalized anxiety disorder (GAD) in three randomized, double-blind, placebo-controlled trials. Since sexual dysfunction (SD) is not well characterized in GAD, a post hoc analysis of these trials was conducted to evaluate the effects of vilazodone on sexual functioning in GAD patients. Materials and methods: Data were pooled from one fixed-dose trial of vilazodone 20 and 40 mg/day (NCT01629966) and two flexible-dose studies of vilazodone 20–40 mg/day (NCT01766401, NCT01844115) in adults with GAD. Sexual functioning was assessed using the Changes in Sexual Functioning Questionnaire (CSFQ). Outcomes included mean change from baseline to end of treatment (EOT) in CSFQ total score and percentage of patients shifting from SD at baseline (CSFQ total score ≤47 for males, ≤41 for females) to normal functioning at EOT. Treatment-emergent adverse events related to sexual functioning were also analyzed. Results: A total of 1,373 patients were included in the analyses. SD at baseline was more common in females (placebo, 46.4%; vilazodone, 49%) than in males (placebo, 35.1%; vilazodone, 40.9%). CSFQ total score improvement was found in both females (placebo, +1.2; vilazodone, +1.6) and males (placebo, +2.1; vilazodone, +1.0), with no statistically significant differences between treatment groups. The percentage of patients who shifted from SD at baseline to normal sexual functioning at EOT was higher in males (placebo, 40.6%; vilazodone, 35.7%) than in females (placebo, 24.9%; vilazodone, 34.9%); no statistical testing was performed. Except for erectile dysfunction and delayed ejaculation in vilazodone-treated males (2.4% and 2.1%, respectively), no treatment-emergent adverse events related to sexual functioning occurred in ≥2% of patients in either treatment group. Conclusion: Approximately 35%–50% of patients in the vilazodone GAD studies had SD at baseline. Vilazodone and placebo had similar effects on CSFQ outcomes in both females and males, indicating a limited adverse impact on sexual functioning with vilazodone. A short animated movie I created as part of an extended project at college. It discusses a particular type of drug that improves your brain's cognition. The video highlights the benefits, risks, and ethical problems associated wih bringing these drugs into society. TMS or ECT for Depression Treatment

Stephen Ross, MD: Psilocybin-Assisted Psychotherapy to Treat Anxiety from Life-Threatening Cancer
As part of The National Center on Addiction and Substance Abuse Speaker Series, in which leading experts present their latest findings, Stephen Ross, MD, Associate Professor and Director of Addiction Psychiatry at NYU Medical Center, discussed his latest findings on using psilocybin to reduce the psychological effects and severe stress that many advanced stage cancer patients face. We spoke with Dr. Ross to hear a little bit more about this fascinating area of research. GIVEN YOUR BACKGROUND IN ADDICTION AND SUBSTANCE ABUSE, WHAT GOT YOU INTERESTED IN EVALUATING THE USE OF PSYCHEDELICS TO TREAT PSYCHIATRIC CONDITIONS, SUCH AS ALCOHOLISM AND END-OF-LIFE ANXIETY? Dr. Ross: In my general psychiatry and addiction psychiatry training, I learned nothing about the history of research with psychedelics within psychiatry and psychology from the 1950s-70s. A colleague informed me of this research in 2006, and when I looked into it, I was surprised to find that the most studied and most promising indication for using psychedelics to treat medical conditions was for alcoholism and terminal cancer-related psychological distress. I found this very interesting, and there appeared to be promising results that were never followed up on because of the passage of the 1970 Controlled Substances Act, which classified psychedelic drugs as Schedule I (indicating “no current medical use” and “high abuse potential”). YOU MENTIONED IN YOUR TALK THAT PSYCHEDELIC RESEARCH, WHICH WAS ONCE BANNED, IS MAKING A COMEBACK. CAN YOU BRIEFLY DESCRIBE WHY THAT IS AND WHAT YOU THINK THE POTENTIAL VALUE COULD BE FOR PATIENTS? Dr. Ross: It is experiencing a resurgence because a small but growing group of academic psychiatrists and psychologists at major academic medical institutions are interested in this line of research and see its potential application to patient care. There could be value in these treatment models if the data definitively show that they work. However, we need much more research to be able to know this. The potential value, if they do work for various problems, is a new model of treatment that is novel (i.e., single to a few doses per treatment episode with sustained clinical benefits). YOU RECENTLY CONDUCTED A TRIAL LOOKING AT THE USE OF PSILOCYBIN ON ANXIETY AND PSYCHOSOCIAL DISTRESS IN ADVANCED CANCER PATIENTS. CAN YOU TELL US WHAT THAT TRIAL INVOLVED AND THE OUTCOMES? Dr. Ross: We tested single dose psilocybin vs single dose placebo to treat cancer-related psychological distress (anxiety, depression). The results are currently being written up, but we found that patients showed dramatic changes in anxiety, depression and orientation toward death, and that a single dose effect may last as long as eight months. THE PSILOCYBIN-ASSISTED PSYCHOTHERAPY YOU HAVE EVALUATED REQUIRES A MALE AND FEMALE THERAPIST TO BE PRESENT WITH THE PATIENT THROUGHOUT THE DAY. IS THIS METHOD NECESSARY FOR THE EFFECTIVENESS OF THE TREATMENT OR IS IT MOSTLY TO INSURE SAFETY? Dr. Ross: This is the model developed by the psychiatrist Dr. Stanislav Grof in the 1960s. Having two therapists in the room is important from a safety perspective, as it is a long day of treatment (eight to 12 hours), and if one therapist gets ill or has an emergency, the other therapist will need to be there. YOU SPOKE ABOUT HOW YOU WERE ABLE TO OBTAIN PERMISSION FROM THE GOVERNMENT TO USE PSYCHEDELIC MEDICATIONS IN YOUR CLINICAL TRIALS. CAN YOU BRIEFLY DESCRIBE? Dr. Ross: There is a path to get approval to conduct this type of research, but it is painstaking and laborious. It requires governmental approval, specifically, an Investigational New Drug application from the U.S. Food and Drug Administration and a Schedule I license from the state’s Bureau of Narcotic Enforcement and the Federal Drug Enforcement Administration. It also requires a local institutional approval from a review board. WHAT DO YOU PREDICT THE BENEFITS MAY BE FROM USING PSILOCYBIN-ASSISTED PSYCHOTHERAPY TO TREAT ALCOHOL USE DISORDERS?
