McMullen, MD about Medications for Depression & Bipolar http://tmsbraincare.com/medications-bipolar-depression-hd-video/ Medications for treatment of Depression & Bipolar disorders Major depressive disorder is also known as major depression, clinical depression, or unipolar depression. The term unipolar refers to the presence of one pole, or one extreme of mood- depressed mood. This may be compared with bipolar depression which has the two poles of depressed mood and mania (i.e., euphoria, heightened emotion and activity). Bipolar disorder (also known as manic depression) causes serious shifts in mood, energy, thinking, and behavior—from the highs of mania on one extreme, to the lows of depression on the other These are of 2 types • Bipolar I disorder involves periods of severe mood episodes from mania to depression. The highs may even require hospitalization of the person. • Bipolar II disorder is a milder form of mood elevation, involving milder episodes of hypomania that alternate with periods of severe depression The depression that people with bipolar disorder experience is generally of a melancholic or psychotic type and therefore more biological in its nature. The depression found in these bipolar patients can be difficult to treat than unipolar depression as the antidepressants can cause them to have greater mood swings. These patients need to be on mood-stabilizing drugs. The antidepressants alone can actually increase the manic episodes and worsen the disorder. Antidepressants: divided into three main classes: • Selective serotonin reuptake inhibitors (SSRIs) - were developed in the 1980's and are the most common prescribed today. They are sold under brand names such as Prozac, Paxil, Prozac, Luvox, Zoloft, Celexa • Monoamine oxidase inhibitors (MAOIs) There are three types of MAOIs, phenelzine,(Nardil) isocarboxazid and tranylcypromine, ( Parnate) and moclobemide.) • Tricyclic drugs (TCAs). (sold as Amitriptyline, Imipramine) For effective control and to limit the side effects a combination can be used such as Wellbutrin (or bupropion). Mechanisms of action are different hence lower doses of both help control side effects. The combination of Zoloft and wellbutruim earned the popular name Welloft. The MAO inhibitors are more effective than the SSRI drugs. But can cause severe weight gain and therefore patients need to work on diet control, else the spike in blood pressure can be dangerous. They are contraindicated with drugs that work on serotonin. Lamictal (or Lamotrigine) was FDA approved for treating depression in 2003.The main advantage being, no sexual dysfunction, no sexual side effects and no weight gain. There is no sedative effect and dosage is generally much less than used for seizures. If any of the above mentioned combinations are ineffective in controlling depression, the thyroid hormone replacement drug Cytopan or Cytomel or Synthroid can be added to any of the above drug. Atypical anti-psychotics such as low doses of Zyprexa, or Abilify, or Seroquel, and the latest Quetiapine can also be prescribed. 150 mg of Lithium, normally prescribe for Bipolar at much higher doses, can be used safely without having to get regular blood checks. It blocks some adrenergic auto and heteroreceptors, increasing serotonin release, and selectively antagonizes serotonin receptors in the central and peripheral nervous system. It also enhances serotonin neurotransmission and blocks the histaminergic and muscarinic receptors. Mirtazapine is not a serotonin or norepinephrine reuptake inhibitor but may increase serotonin and norepinephrine by other mechanisms of action. Side Effects: Constipation, dry mouth, weight gain, increase appetite, somnolence, sedation, sleepiness, weakness, distrurbance in thinking, peripheral edema, increased blood pressure. When Discontinuing Use: Mirtazapine and other antidepressants may cause a discontinuation syndrome upon cessation. A GRADUAL and SLOW reduction in dose is recommended. Effects of sudden cessation of treatment with mirtazapine may include depression, anxiety, panic attacks, vertigo, restlessness, irritability, decreased appetite, insomnia, diarrhea, nausea, vomiting, flu-like symptoms such as allergies and pruritus, headaches and sometimes hypomania or mania. It can go so far as to cause suicidal thoughts, so please, be careful!! AKA not good!!! PLEASE always been under the care of a great doctor and NEVER stop taking a drug suddenly without guidance from your doctor. I just wanted to share all the medications I am taking. Can be aggravating but I'm trying to stay the course this time. Last year when they put me on Lexapro I got off of it pretty quick. And I stopped taking Zrytec after 2yrs straight. In addition to what I take now (Zoloft 100mg, Klonopin 0.25mg half pill when needed, Prilosec 40mg, Allegra 180mg, 2 nasal sprays). I get a lot of questions about bipolar medications and, in this video, I answer them; including how they changed my life for the better. If you’re recently diagnosed, one of the biggest problems is finding the right medication for bipolar disorder that works specifically for you. Unfortunately, this can be extremely difficult and exhausting. After my diagnosis of bipolar 2, the first step was trying various medications to see what worked and what didn't. Unfortunately, the side effects of many of these bipolar medications can be severe, such as weight gain, extreme drowsiness, and other effects that hinder a person's ability to function. There was a point that the bipolar medications seemed to only increase my depressive and emotional behavior. I was incapable of communicating properly with those around me. Finally, I went to my doctors and demanded to find the right routine of medications. It took me around 5 years to find a steady regimen of medications that efficiently managed my bipolar disorder without dampening my personality and increasing my weight. I still experience highs and lows on medication, however, I have more control over it, whereas before my condition was in control of me. Hopefully, in the future, research will uncover new treatment methods with fewer side effects. Until then, do not lose hope and continue communicating with medical professionals seeking the right routine of bipolar medications for you. In this presentation, you will learn six hopeful and innovative approaches for helping people with depression to get some traction to begin to come out of it. Because depression has multiple causes and contributing factors, there are many ways to shift it. These approaches draw on respectful, strength-based models as well as the latest brain plasticity research. This research casts doubt on traditional explanations of depression and shows a whole array of new angles for treating it. You will learn about tools that you can use immediately to help chronic and acute clients suffering from depression. The presentation will be filled with compassion, learning, fun and possibilities. Bill O’Hanlon, who has suffered and recovered from depression, is a lively presenter with a fresh take on this topic. Serotonin, or 5-hydroxytryptamine (5-HT), is a neurotransmitter involved in many brain and body functions and is commonly known as the substance of well-being and happiness. Serotonin is produced in specialized neurons found mostly in the Raphe nuclei located along the midline of the brainstem. The axons of these neurons form extensive serotonergic pathways that reach almost every part of the central nervous system, including the cerebellum and the spinal cord. This is why it’s not surprising that serotonin is implicated in a vast array of brain functions, including sleep and wake cycle, appetite, mood regulation, memory and learning, temperature control, … among others. Serotonin is synthesized from the amino acid tryptophan and is stored in small vesicles within the nerve terminal. When a serotonergic neuron is stimulated, serotonin is released into the synaptic cleft where it binds to and activates serotonin-receptors on the postsynaptic neuron. Serotonin action is then TERMINATED via removal of its molecules from the synaptic space. This is accomplished through a special protein called serotonin-transporter. Low levels of serotonin in the brain have been associated with depressive disorders and current treatments for depression aim to increase these levels. The most commonly prescribed medications, called “selective serotonin reuptake inhibitors”, or SSRIs, act by blocking serotonin reuptake by the transmitting neurons. This results in elevated levels of serotonin in the synaptic space and its prolonged action on the receiving neuron. The SSRIs have developed into the drugs of choice because they produce fewer side effects thanks to their selective action on serotonin alone and no other neurotransmitters. Unfortunately, because serotonin is involved in a wide range of brain functions, the side effects remain significant and may progress to a potentially dangerous condition known as "serotonin syndrome". This syndrome is generally caused by a combination of two or more drugs used to raise the serotonin levels in the brain. If the medications are not discontinued, the condition may become fatal. Nonpharmacologic methods of raising brain serotonin have shown promising results in recent studies. It has been suggested that positive mood induction, either self-induced or due to psychotherapy, correlates with INCREASED serotonin synthesis in the brain. The interaction between serotonin synthesis and mood may therefore be 2-way, with serotonin influencing mood and mood influencing serotonin. Other methods include exposure to bright light and tryptophan-rich diets. To note, however, that serotonin-rich food such as bananas would NOT work because serotonin, unlike tryptophan, can NOT cross the blood brain barrier. Finally, although it sounds like a cliché, physical exercise maybe the most effective and safest way of improving mood. Several studies suggest that serotonin levels are increased with vigorous physical activity and that these elevated levels are maintained for several days after the exercise.

You wanted to know if there are any safe medications to take for depression during pregnancy. And this is an excellent question for your OB provider. I suggest talking to them, and after discussing your thoughts, and feelings, and concerns, they'll determine a treatment plan. Sometimes that involves counseling, sometimes it involves medication. And if it does involve medication, they'll be able to give you tailored advice about what would be best for you and what would be safest. When it comes to medications during pregnancy, we can't ethically study them and make sure that they're going to be safe, so any medication that is taken, is taken with some risk. The information that we have is usually gathered in retrospect. We know that there are women who have taken certain medications, and we know that there may or may not be certain outcomes that are associated with that. So your doctor will discuss the lowest risk options with you and determine what would be best.
